If you're eligible for Medicaid and you're living with HIV/AIDS, homeless, or transgender, you have the option to enroll in an HIV Special Needs Plan (SNP). These plans cover the same services as other Medicaid plans but also provide specialized services for individuals with HIV/AIDS.

There are multiple SNPs available for you to choose from. By joining a SNP, you'll have access to an HIV specialist primary care provider, HIV care coordination services, information on medications and side effects, treatment adherence support, and other specialty services.
